Your intake, your starting point.
Before the program begins, we need to know where you are. Not where you hope to be, not where you think you should be. Where you actually are right now.
What this intake is
This intake does three things. It helps us assign you to a team with good balance. It gives you a personalized baseline report at the end, so you leave with something useful. And it is the first act of honest self-assessment this program will ask of you. Everything here is private between you and your facilitator.
How to answer
Be truthful. No one is scoring what you report. The more accurate your answers, the more useful the program will be. If a question feels hard, that's often a signal the honest answer is the important one. If you need to skip something, skip it, but aim for honesty over comfort.
Time: About 20 to 25 minutes. You can save and come back later.
Sections: 9 sections covering your current situation, study habits, learning profile, and honest reflection.
Result: A personalized baseline report you'll review in week 1.
The basics
Contact information and logistics so we can reach you and schedule you into a team.
Accommodations and access
Optional. This information is held in confidence and used only to support your participation.
Your current course
The specific situation we're helping you recover from. Accuracy here matters because it shapes everything downstream.
We're asking so we understand your context, not because we'll reach out. Your course situation remains yours.
Where you stand
Use whatever form is on your gradebook. Percentage, letter, or number.
This is the institutional minimum to not fail. Check your syllabus if you're not sure.
Many nursing and pre-health programs require a higher grade than passing. For example, many nursing programs require a B or better in A&P even though the course passes at a C. If your target program has a specific grade requirement, put it here.
Optional. Helps us understand the stakes so we can triage accurately.
What, when, and what it covers if you know.
Your history with this course
Your journey so far
What you've already tried in this course. Not what worked, just what you've attempted. The truth here helps us understand why you're in recovery.
What's actually broken
Students fail for different reasons. Identifying your primary pattern helps us focus your recovery. Answer each question honestly. We'll show you your pattern at the end.
Rate each statement: how true is it for you right now? 1 means not true at all, 5 means very true.
