NGN — Unfolding Case Studies
Each case has 6 items walking through the CJMM steps.
Structure: Each case includes a medical record (nurses' notes, vitals, labs, orders) followed by 6 items — one per CJMM step: Recognize cues → Analyze cues → Prioritize hypotheses → Generate solutions → Take action → Evaluate outcomes. Item type varies per step (highlight, matrix, cloze, extended multi-response, etc.).

Every question gets: (1) why the correct answer is correct, with the underlying pathophysiology or safety principle; (2) why each distractor is incorrect — addressing the specific misconception it targets; (3) the NCLEX tag block so students learn the blueprint as they practice.
NCLEX item-writing rules applied automatically: no "all of the above" / "none of the above"; negatively-worded stems only when clinically necessary (and clearly marked); SATA has 5-6 options with 2-4 correct; priority questions phrased with "first," "initial," "most important"; MC answer key distributed evenly across A/B/C/D.

Gap Finder Mode
Build a diagnostic exam to surface knowledge gaps. Broad coverage instead of deep — one or two questions per subtopic, no unfolding cases, and the answer key includes a Gap Analysis Report identifying weakest content areas.
Scope
When enabled: Apply/Analyze cognitive level only (no Remember items). Unfolding cases disabled. Each question tagged with its specific micro-concept. Output includes a Gap Analysis Report with study priority matrix and review pointers by gap area. Sticky topics still get extra weighting in either scope.
